Working as a Clinical Pharmacist in the medical wards (cardiology, surgical, Respiratory, Palliative) alongside daily dispensary slot as per the rota.
• Validating and screening patients' prescriptions by using different software such as: eMed, Ascribe, Single view.
• To ensure trust guideline is followed at all times.
• Have a good command of different computer software such as: eMed, Ascribe, Single view, used for validation and screening of patients' prescriptions.
• To ensure safe and timely supply of new medicines.
• To ensure accuracy of medicines reconciliation on admission.
• Play an important role in medicine optimisation.
• To liaise with medical staff/Nursing staff to ensure safe prescribing & administration
• To Complete patients' discharges on ward areas.
• To identify patients on narrow therapeutic index drugs and make necessary adjustments.
• To identify patients with complex pharmaceutical care problems and discuss them with a more experienced pharmacist if appropriate.
• Organise and delegate workload undertaken by preregistration pharmacist technicians, dispensers, and other staff members.
• Provide a comprehensive pharmacy clinical service to adult oncology/rheumatology inpatients and outpatients.
• Overseeing clinical trial set-up in pharmacy and ensure they are dispensed accurately and safely.
• Completes appropriate documentations for example: non-formulary requests, intervention data base.
Also, I occasionally work as a locum pharmacist in community is well where my job responsibilities are as follow:
• To ensure all prescriptions are valid clinically & legally
• To ensure that different treatments are compatible
• To supervise the preparation of any medicines (not all are supplied ready made-up by the manufacturer)
• To offer flu vaccine service
• To keep a register of controlled drugs up-to date
• liaise with doctors about prescriptions
• To sell/supervise safe supply of over-the-counter medicines
• To advise public on the treatment of minor ailment & any adverse side-effects of medicines or potential interactions with other medicines/treatments
• To ensure medicines supplied in dosette boxes are safe and are clinically and legally valid
• To undertake Medicine Use Reviews (MUR), an advanced service to help patients understand how their medicines work and why they have to take them
• To provide support through the New Medicine Service (NMS) to patients starting certain medicines to treat long-term conditions
• To manage a needle and syringe exchange
• To offer support in specialist health checks, such as blood pressure
• keep up to date with current pharmacy practice, new drugs and their uses
• To provide medicines to general public referred by 111 service and process them as onward referral where relevant by using CPCS and Pharma-outcome service
